UHG's NSW Police Force Pre-employment Medical Examination Program

UHG is proud to announce its partnership with NSW Police (the largest Police Force in Australia) in the provision of a pre-employment medical screening program for applicants seeking employment with NSW Police.

As an applicant you will be required to follow the process outlined below to complete your pre employment medical assessment. For information pertaining to the inherent requirements of working as a police officer please click on Link

Step 1:

Download and print off all assessments forms via the links below. You will need to fill in the Medical History form. Take the completed Medical History form and all other forms to the medical consultation appointment which will be organised at the completion of this process.

Step 2:

You are required to pay for your medical consultation appointment before your appointment can be made. The cost of this consultation is
$ using a valid VISA or MASTERCARD. AMEX will incur an additional 3% surcharge. You will be provided with a Tax Invoice of this transaction via your nominated email address.

Once payment is successful, one of our customer service team members will contact you on your nominated mobile to organise your medical consultation appointment.
