Cogent Thinking's NSW Police Force Pre-employment Medical Examination Program

Cogent Thinking is proud to announce its partnership with NSW Police (the largest Police Force in Australia) in the provision of a pre-employment medical screening program for applicants seeking employment with NSW Police.

As an applicant you will be required to follow the process outlined below to complete your pre employment medical assessment.

Step 1:

Please review the below documents. Download and print off all assessments forms that may be relevant to you via the links below.

• NSW Police Medical Assessment Clinic Locations
• NSW Police Medical History Instruction Sheet
• NSW Police Audiometry Form

  (Applicants who have a known hearing condition and/or use hearing aids are to have a separate hearing assessment completed by a qualified Audiologist only)

• NSW Police Vision Assessment

  (Applicants who declare use of glasses and/or contact lenses (either full time users or part time users) are to have a separate vision assessment completed by a qualified Optometrist only)

• NSW Police Pathology Referral

If you have vaccination documentation, test results and doctor/specialist reports that may assist in your assessment, please bring these to your appointment.

Step 2:

You are required to pay for your medical consultation appointment before your appointment can be made. The cost of this consultation is
$ using a valid VISA or MASTERCARD. AMEX will incur an additional 3% surcharge. You will be provided with a Tax Invoice of this transaction via your nominated email address.

Once you have paid for your assessment, Cogent Thinking will contact you to schedule your appointment and provide instructions on how to prepare for your appointment.
